The only Legal one must be copied directly from the Code Book
NFPA #72

There is no ONE legal form... Unless your AHJ has deemed otherwise in his
jurisdiction.
Table 7-3.2 Testing Frequencies
Figure 7-5.2.2 Example of an inspection and testing form.

Keyword is EXAMPLE

But that's the one I'd direct him to as well...

I have a good one modeled from NFPA 72. Maintain a pre-filled in copy for
each client. Each time you go for an inspection, the basics are already
recorded and you can spend your time doing the inspection.

I will be glad to send it to you, but you are using an invalid return
address.
Regards,
Allan

Is yours an Excel or Word document? Or just a copy of something on paper?
I'd be interested in looking at it...

Its a word document that I typed. What I do is fill in all the fixed data,
like panel type, special instructions, correct CS phone number (we use
several different stations), where everything is located, a space for each
device to be tested, etc, etc. I even list each battery separately so that
the load test and mho test is by individual battery. When it is time to do
the inspection, then we print it, all the time wasting paperwork is already
done... professionally. Our labor time on the job is almost all inspection
only.

After the inspection, we make a copy of the current inspection report and
send the copy to the customer with their invoice. When they see how much
work is involved in an inspection of their system, the never balke at the
price. The other copy stays in my file for the customer.

I will be glad to send you a copy, but you need to give me a valid e-mail
address.
Regards,
Allan
