Hello all :)

I have a battery-powered LED lamp. The battery seemed to have failed and the lamp wouldn't turn on.
I'm looking to re-wire the LED to a plug-in power supply, but not sure which Voltage/Amp configuration I should use.

It uses a 18650 (3.7V) battery with 4000mAh capacity.
The lamp uses a 5V-1A USB-C charger, and the LED is rated at 1.5W

Appreciate any help possible!

There will be a charge and LED current control circuit in the black shrink tube bag.
You can't connect an LED directly to a power supply, you need controlled current. This is what the electronics in the "bag" does.

The battery seemed to have failed
Have you measured the battery voltage? The fail could also be within the electronics or even the LED may have failed.
If the battery is not a total fail, the LED should still light up with the power supply (aka charger) connected to the lamp.
