sine wave conversions

Hello, I am reading the voltage output of the attached circuit with the adc from an pic18f8520 i would be really grateful if someone can help me with the formula for the circuit, a relationship between the input and the output voltage, all my attempts on finding out a formula failed.

I dont know if it help, its not resulting from the the OpAmp logic but its a close relationship between input and output

Out=(1/12)in + 2

i would want to take in account for the capacitive reactance also
