Too much voltage on the capacitor, poor regulation...as kip said...

You are using the correct grade of capacitor? 'Ordinary' ones won't do.

You beat me to it. That would be my first guess for a likely cause.

Check the forward converter FET`s, and strapping diodes(if any) or it
may keep happening.
