Most 5VDC input DC-DC converters only have a maximum output of +15/-15. MC34063 http://www.onsemi.com/pub/Collateral/MC34063A-D.PDF should be able to do it quite easily, just an inductor and a few other components. theres various applications on the data sheet and also other application notes.
